I, We, the undersigned do hereby agree to employ the services of Intelliquest Investigations,  private investigation agency, duly licensed under the laws of the state of Arizona, for the purposes of conducting: (Please read contract in full before submitting form)

It is agreed and understood that I, We, shall be solely responsible for the compensation to Intelliquest Investigations at the agreed rate of $75.00 per hour ($80.00 per hour for surveillance-2 vehicles) per investigator, exclusive of all expenses incurred during or arising from the investigation.  It is further agreed and understood that should I, We, desire additional investigation/surveillance, apart from those described above, then I, We, shall compensate Intelliquest Investigations at the rate stated in the current schedule of fees described in the fees page of this website or as otherwise agreed upon. 

It is agreed and understood that the above fee shall be due and payable within 10 days of notice.  In the event of default in payment of sums due hereunder and if the agreement is placed in the hands of an attorney at law for collection, I, We, agree to pay all costs of collection including but not limited to a reasonable attorney's fee.

In consideration of the foregoing terms and conditions, I, We, understand that Intelliquest Investigations shall under its best efforts, investigate the matter set forth above and provide me, us, a written report of its findings; and while Intelliquest Investigations agrees to make a reasonable effort to ensure the accuracy of the information provided, it is not responsible for the content or accuracy of information originating from a third party or agency.

I, We, hereby agree to allow said investigative agency to conduct the investigation at its sole discretion.  It is agree and understood that Intelliquest Investigations will conduct its investigations in strict compliance with The Privacy Act, The Freedom of Information Act, The Fair Credit Reporting Act, and applicable local, state, and federal laws; and that I, We, assume full responsibility for obtaining any and all required records or information releases and authorizations from job applicants when necessary.

I, We, Our, My heirs, beneficiaries, devisees, legatees, administrators and assigns further agree to indemnify and hold harmless Intelliquest Investigations and/or its agents and employees from any and all actions, causes of actions, claims, damages and demands of whatever type wherever situated arising directly or indirectly from their investigation which I, We, have requested above.

This agreement shall be terminated immediately upon receipt of written notice by either party. (Fill in form and click submit button)
